Three serious forklift incidents – one fatal
In February 2022, a worker suffered serious leg injuries after being struck by a forklift. Early investigations suggest the man was walking next to the forklift to prevent a load of glass panels from swinging. In March 2022, a worker suffered fatal injuries when he was struck by a forklift, which early investigations indicate was moving and carrying a load. In May 2022, a worker suffered serious injuries to his lower legs when he was struck by a counter-balanced forklift. Early investigations show the worker was walking behind the loading dock area of the warehouse when the forklift reversed out of a truck into the warehouse and struck him. Investigations are continuing.

Man seriously injured when elevating work platform overturned
In January 2022, a man was operating a self-propelled scissor lift which overturned leaving him with serious head injuries. Early investigations found the worker was alone, replacing rivets on metal flashings in an indoor sports facility.

Workers struck by moving vehicles
In January 2022, a man died after being hit by a truck a worksite. The worker was part of a crew laying out power lines on the side of the road. Early investigations indicate the man may have been talking on his mobile phone when hit by the reversing truck. In December 2021, a worker was seriously injured by a reversing road roller during asphalt laying operations at a road work construction site. Initial enquiries indicate the worker was testing the asphalt near the roller which struck him while compacting the road surface. In November 2021, a worker also lost his life after being hit by an unmanned truck which he may have been warming up.

Worker fatally crushed after being hit by moving vehicle
In August 2021, a worker was fatally injured when he was crushed between his truck and a concrete column. Early investigations indicate he had positioned the truck in the loading dock to wash the back out and clean it. It appears a timber pallet was used as a wheel chock to prevent the truck rolling. The driver was seen to go behind the truck where it is reasonably believed he was repositioning the pallet, when the truck rolled back and trapped him.

Truck driver exposed to phosphine gas
In July 2021, a truck driver began feeling very unwell after transporting grain to a feedlot with his semi-trailer. It appears the worker was exposed to a fumigant that produced phosphine gas which is commonly used to control insect infestations in grain.

In January 2021, a water truck operator was working on a civil construction job when the vehicle rolled over. He suffered fatal injuries after being thrown from the cab and trapped underneath the truck.

In January 2021, a farmer died when the tractor he was operating struck a log and rolled. The farmer had been using the tractor to pull out a bogged car and do some spraying. He was thrown from the tractor when it rolled and was trapped underneath the rear mounted chemical tank.

Worker falls from forklift
In December 2020, a worker suffered serious injuries when he fell from a forklift. Early investigations indicate he was attempting to repair a jammed roller door. The man was working from a stillage container being used as a makeshift work box and had been lifted on the tines of a forklift. It appears that as the worker moved to one side of the crate it overbalanced causing him (and the crate) to fall almost 5 metres.

Person falls from tank
In November 2020, a man sustained serious head injuries after falling from a water tank that had just been put in place by a vehicle loading crane. It appears he was standing on the tank close to the crane.
