    In March 2023, a worker sustained fatal crush injuries when the tractor he was operating veered off the path over an embankment and rolled. Early investigations indicate that the worker was not wearing a seat belt. For reasons yet to be established, the tractor has veered off the path over an embankment rolling over.

    In April 2023, a young worker suffered serious hand injuries after an incident on a construction site. Early enquiries indicated the worker was operating a drop saw to cut noggins when for reasons yet to be established his hand came into contact with the moving blade, causing significant amputation injuries.

    In March 2023, a truck driver suffered fatal injuries after becoming trapped between the tipper body and chassis of a semi-trailer tipper combination. Early investigations indicated the truck driver appears to have been inspecting one of the tipper trailers at the time.

    In February 2023, a roofer was fatally injured when he fell over 5 metres from the roof of a shed under construction. Early investigations indicated he was installing roof sheeting on a portal framed shed when for reasons yet to be established, he lost his balance near the live leading edge and fell to the ground.

    In October 2022, a worker suffered serious burns to his upper body after being engulfed by a fire at an engineering workplace. Early investigations revealed he was welding a metal plate to repair a leak to a tank, when an explosion occurred which resulted in a fire.

    Four injured in workplace explosion

    In November 2021, one worker was seriously injured while three other workers received minor injuries following a workplace explosion and fire. Early enquiries indicate an “extractor” has exploded. This machine is used to extract rosin and turpentine from pine wood chips by spraying hot turpentine over the wood chips in a process known as solvent extraction. For reasons yet to be established, an explosion has occurred. Investigations are continuing.

    Falls from height result in serious injuries

    Two recent incidents where workers have suffered serious injuries have highlighted the dangers of falls from height. In August 2020, a man sustained serious injury when he fell approximately 2m from a ladder while working at a residential property. Early enquiries indicate he was using a ladder to access the roof area. In a separate incident a month late, a worker fell approximately 3.4m at a construction site. It appears he was helping unload building materials from an elevated position of a partially built structure.

    The purpose of this safety alert is to highlight the potential for degradation of fibreglass ladders that can lead to a structural failure.

    Worker fatally crushed by skid steer loader

    In February 2019, a worker was killed while repairing a skid steer loader. Early investigations indicate the safety prop was not engaged to ensure the bucket arms could not be lowered. The worker inadvertently activated the controls and was crushed by the arms. It appears the mechanisms which enable the hand controls to be isolated were inoperative due to general wear and tear on the machine. Investigations are continuing.
