    Person electrocuted digging trench

    In May 2023, a person was electrocuted while attempting to repair a water leak in an underground pipe. Early enquiries indicated that the person unearthed a black poly water pipe believed to be the water pipe that required repair, and then cut into it using conduit shears.

    Worker injured while inflating truck tyre

    In October 2023, a worker sustained head and chest injuries when an inflated truck tyre exploded. Preliminary information suggested a truck tyre exploded when the worker was fitting a re-inflated tyre to the truck. It appeared the inside of the tyre wasn’t inspected prior to inflating and inflation didn’t occur inside a tyre cage.

    In June 2023, a worker sustained serious head injuries when he was operating a stand on pallet mover. Initial investigations indicated the worker was operating the pallet mover in a large storage freezer travelling beneath a low-lying pallet racking system. It appears his head collided with the steel rack cross supports.

    Four workers injured in gas explosion

    In June 2023, four workers were injured in a workplace gas explosion and fire. Early enquiries indicate they were clearing a gas line by purging a liquefied petroleum gas (LPG) boiler line, creating a flammable atmosphere in the room. For reasons not yet known, a brazing torch ignited the atmosphere, resulting in an explosion and fire.

    In October 2022, a worker suffered serious burns to his upper body after being engulfed by a fire at an engineering workplace. Early investigations revealed he was welding a metal plate to repair a leak to a tank, when an explosion occurred which resulted in a fire.

    Fatal incident involving garbage truck

    In November 2021, a mechanic died after being trapped in the back of a rear-loading garbage truck.Early investigations indicate the man had climbed into compacting section of the garbage truck to fix a problem when for reasons yet to be established he was trapped and suffered fatal injuries. Investigations are continuing.

    In November 2020, a man sustained serious head injuries after falling from a water tank that had just been put in place by a vehicle loading crane. It appears he was standing on the tank close to the crane.

    Falls from height result in serious injuries

    Two recent incidents where workers have suffered serious injuries have highlighted the dangers of falls from height. In August 2020, a man sustained serious injury when he fell approximately 2m from a ladder while working at a residential property. Early enquiries indicate he was using a ladder to access the roof area. In a separate incident a month late, a worker fell approximately 3.4m at a construction site. It appears he was helping unload building materials from an elevated position of a partially built structure.
