Overhead powerline incidents
In October 2021, a man suffered electric shock and severe burns while installing gutter guarding on the roof of an industrial shed. For reasons yet to be established, the worker came into contact with a nearby 11kV single wire earth return (SWER) while he was on the roof. In November 2021, a worker suffered serious burns and another minor injuries when a mobile concrete placing boom contacted an 11kV overhead powerline at a residential construction site. Initial enquiries indicate the mobile concrete placing boom was operating at the time of the incident.

One worker killed and six injured when powerline arced to harvester
In July 2021, one worker died and six others received electric shocks when a harvester came close enough to an overhead powerline to cause an electric arc. Initial investigations found the powerline had dropped approximately 3.6m from its original height because the stay wire on one of the poles had broken, potentially due to corrosion.

DC isolators – consequences of cancellation of a number of certifications
Direct current (DC) isolators used in photovoltaic solar systems in household installations are classified as high risk (level 3) electrical equipment in the Electrical Equipment Safety System (EESS). They are required to be registered on the EESS national database before they are supplied in Queensland. This registration requires using a valid certificate of conformity.

Arc flash incidents
In January 2021, two electrical workers suffered burns following an arc flash. They were working in a switch room with an LV main switchboard. Initial enquiries indicate electrical work was being done on the switchboard metering when the incident occurred.

In November 2020, a man sustained serious head injuries after falling from a water tank that had just been put in place by a vehicle loading crane. It appears he was standing on the tank close to the crane.

Falls from height result in serious injuries
Two recent incidents where workers have suffered serious injuries have highlighted the dangers of falls from height. In August 2020, a man sustained serious injury when he fell approximately 2m from a ladder while working at a residential property. Early enquiries indicate he was using a ladder to access the roof area. In a separate incident a month late, a worker fell approximately 3.4m at a construction site. It appears he was helping unload building materials from an elevated position of a partially built structure.

Workers burned by arc flash in switchboards
In October 2018, an electrical worker received burns to his hand, neck and face from an arc flash while he was terminating cables running to a switchboard.In September 2018, an electrical worker received burns to his hands when his pliers shorted two phases while attempting to fit a three phase circuit breaker to a live switchboard. Investigations into both incidents are continuing.
