Lodging a review
There are a few ways you can lodge an application for review:
- online (If you choose to complete the application online, please note the application form cannot be saved once it has been started and the application form will time out if a section of the form is open for longer than an hour. If this happens your data will not be saved or sent to the Office of Industrial Relations.)
- Alternatively you can print out a copy of the Application for claim review form (PDF, 0.2 MB) or the Application for a premium/policy review form (PDF, 0.18 MB) and send to the Review Unit via fax, email or mail. Please ensure you complete all sections of the form, provide your grounds for review, attach your supporting documents and sign the form.
- If you would like a copy of the application for review sent via mail or email, please contact the Review Unit.
