Worker killed in trailer rollover
In September 2023, a worker was killed as material unloading activities took place on a construction project. A dog trailer that was part of a rigid truck and single dog trailer combination, rolled onto its side and pinned the worker underneath.

Snorkelling fatalities
In November 2023, a member of the public was participating in snorkelling activities when for reasons yet to be established, was removed from the water in an unconscious state but later passed away. In December 2023, another member of the public was recovered from the water and also passed away after also participating in recreational snorkelling.

Fatal fall from boat in marina yard
In August 2023, a man died when he fell while working on a boat on a hardstand area at a marina. Early investigations indicate he fell to the ground after falling from a ladder at the rear of the boat.In October 2023, a worker suffered serious leg and foot injuries after being struck by a gantry crane wheel. Initial enquiries indicate he was using a jackhammer close to the crane’s rail system when the crane began operating and one of its wheels struck him.

This safety alert highlights the risks associated with delivering concrete to mobile and static concrete pumps in the construction industry. The alert is for workers, employers, builders, owners and operators of concrete delivery trucks, concrete pumping units and concrete batching plants.

Bridge and gantry crane serious incidents
In September 2023, two young workers were injured when a bundle of steel lengths weighing over one tonne fell from a bridge crane (overhead travelling crane). Initial investigations indicate a lifting chain slipped off one end of the bundle of steel while it was being moved using a dual hoist bridge crane inside a warehouse. The bundle fell, striking a worker and a second worker was injured while assisting the first.

Person electrocuted digging trench
In May 2023, a person was electrocuted while attempting to repair a water leak in an underground pipe. Early enquiries indicated that the person unearthed a black poly water pipe believed to be the water pipe that required repair, and then cut into it using conduit shears.

Four overhead powerline incidents involving plant and machinery
In May 2023, a worker was seriously injured after receiving an electric shock at a worksite. Initial enquiries indicated a concrete box culvert was being moved onsite by a pick-and-carry mobile crane with the worker helping. It appears the worker was touching part of the concrete box when the crane came into contact with overhead powerlines.

Worker’s foot injured in auger incident
In October 2023, a worker sustained significant injuries to his foot when it became trapped in an auger. Initial enquiries found that the worker was clearing a blockage in a blasting cabinet return system (consisting of conveyors and an auger) when his foot came into contact with the auger.
