If you wish to report a workplace concern relating to a workplace health and safety issue use the Make a complaint or raise a concern about a work health and safety issue online form.

Ask WHS/ESO a question

If you have an enquiry regarding a safety topic, form, licence, equipment or a general health and safety issue use the WHSQ Enquiry online form.

Notify of an incident at the workplace

Depending on the seriousness, you may need to notify Workplace Health and Safety Queensland (WHSQ) about a workplace incident. Find out which incidents you are required to report before notifying of an incident.
